INSTRUCTIONS FOR USE OF AQUALUNG/SUUNTO SERVICE REQUEST FORM:
- Complete the form by following the prompts.
- Submit the form to Calypso Divers and we will send you an RM number by email which we request you place on the outside of your shipping container.
- Send your package to:

- We suggest that you insure your computer for the retail replacement value of the computer. If your are unsure what value to assign, please call and we will assist.
- Package your computer to protect it during the shipping process. For helpful suggestions on how to pack the computer refer to www.ups.com/content/us/en/resources/prepare/index.html.
- If you call to inquire about the status of your computer repair please reference the RM number to help expedite the status search.
- If you are sending a computer which utilizes a sending unit and suspect communication issues please include the sending unit.
- If you are requesting warranty service you must attach a copy of the purchase invoice which indicates the following:
Computer was purchased from an authorized Aqualung/SUUNTO Dealer, and You are the original purchaser. The invoice must also show date of purchase.
TERMS AND CONDITIONS
1) Responsibility for proper packaging and shipping of your equipment is important to prevent damage while in transit. Please package your equipment carefully. Once the equipment arrives at Calypso Divers, Inc. we will be responsible for proper handling and care. If equipment was damaged during shipping, we will provide you with the information for filing a claim with the shipper. We suggest you refer to http://www.ups.com/content/us/en/resourses/prepare/index.html
for helpful hints for preparing your equipment for shipping.
2) Claims for warranty service for AQUALUNG/SUUNTO Equipment require that a photo copy of the original receipt. Warranty service conditions are stated in your owners manual. Warranty service cannot be preformed without:
- a copy of the purchase receipt
- you must be the original purchaser
- the service requested is as a result of a manufactures defect that occurred within the warranty period.
3) I/ we authorize you to charge my credit card for:
- The cost of parts, labor as currently listed on our website. (Aqualung Dealers can reference their Dealer Manual in the warranty section)
- The cost of return shipping and handling using the method you have selected on this form will be used.
- If additional parts and labor are required as determined by our inspection will obtain permission prior to proceeding.
- Shipments outside the continental United States require pre authorization via fax or e mail.
All parts and labor are warranted for service for a period of 90 days from date of service
